How clinicians use it
Most of a child's session happens in play. This pack helps you describe that play as what it is clinically: a targeted intervention, an observed response, and measurable movement toward a goal.
Built for
- Play, art, sandtray, and talk-based sessions with children and teens
- Insurance, Medi-Cal, school-based, and agency documentation
- Supervisors teaching associates and trainees to write goal-linked notes
A simple flow
- Assess and pick a diagnosis you can support with observed symptoms and functional impairment.
- Write 2–4 goals with measurable objectives (goal bank, pp. 11–12).
- After each session, translate the play into intervention + response language (pp. 3–4).
- Draft the note with the SOAP or DAP prompts (pp. 5–7) — tie every session to a goal.
- Rate impairment and progress at regular reviews (pp. 8–9, 14).
- Close with a discharge or transition summary (p. 15).
Scope
This pack offers writing prompts and examples only. It isn't legal, billing, or coding advice, and it can't make a service billable. Documentation must be truthful and reflect what actually happened. Follow your licensing board, your agency, and your payer's or county's current standards (for Medi-Cal, your county mental health plan or managed care plan) — those override anything here.
